Restoration of testes

Introduction: Transverse testicular ectopia associated with persistent Mullerian duct syndrome (PMDS) is a rare form of male pseudohermaphroditism that is usually discovered unexpectedly during surgery for cryptorchidism.

Case Presentation: An adult male presented with complaints of primary infertility and inguinoscrotal swelling. An ultrasound scan revealed a hernial defect protruding into the scrotum on one side, and the testis was absent on the contralateral side. Intraoperative findings showed an undescended testis present on the left inguinal region, along with the uterus and both fallopian tubes. Histopathological and karyotyping findings confirmed the diagnosis of persistent Mullerian duct syndrome.

Conclusion: It is crucial for the operating surgeon to be familiar with this rare disease condition to increase the chances of accurately diagnosing and appropriately dealing with Mullerian duct remnants.
